Health Coverage Tax Documents & Payroll Information
As a result of the Paperwork Burden Reduction Act, Burkes Mechanical, Inc. will not automatically furnish the Form 1095-C, Employer-Provided Health Insurance Offer and Coverage, for the 2025 tax year.
You have the right to receive a copy of your Form 1095-C upon request. To request a copy of your 1095-C or if you have any questions regarding this matter, please contact the payroll department using one of the following methods:
